Please Note:

We are a specialist office, and it is patient's responsibility to check if a referral is required by insurance to be seen.

Insured Patients:
We will bill your visit directly to your insurance provider if you supply an updated proof of insurance at each visit. Please note that the insurance policy is between you and your insurance provider. We are happy to file the claim for you, but the balance is your responsibility if your plan does not pay after 30 days. Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us.  Any copay amount is due at the time of your office visit.

Out-Of-Pocket and Uninsured Patients:
The first visit consists of a $150 upfront cost. $100 will then be collected for every follow-up visit. Allergy skin tests will be $5 per prick, and intradermal tests will $8 per prick. Each allergy shot visit will be $15.
